Parents,
Please read the following adjustments in our procedures to request a compliance letter for your student to take the permit test.
You must have a scheduled appointment to take the written test before requesting this form. If you have yet scheduled it, please go to http://kentuckystatepolice.org/driver-testing/ first.
To request a form please email Christina.Tarbet@oldham.kyschools.us Forms must be requested at least one school day in advance. They will not be available on demand. It takes a little time on our end to properly prepare the form and the busyness of a school doesn’t always allow us to do it in the moment of the immediate request.
For forms over the summer, please call the office at 228-0158 due to limited staff availability and limited summer office hours.
We will have the permit forms available for pick up beginning at 7:45 the next business day in the NOHS front lobby. Our office hours are Monday-Friday 8:00am to 4:00pm. The form expires after 60 days. Again, please do not request this form unless you have a written test scheduled within the next 60 days.

NOTE: School breaks and holidays tend to be a time during which students get their permits, but these are also times during which staff members may not be on campus to issue the form, so please plan accordingly.
These forms are approved based on the condtions set forth in the KY NO Pass No Drive Law. Students must maintain their grades and school attendace to keep their permit or license. NOHS completes grade and attendace checks at the end of each semester and reports students as directed by the NPND Law to the KY Transportation Department. Students that no longer qualify will have their permit or license suspended. NOHS will not re-evealute grades or attendace until the end of the next semester.
